Vertére Access Request Form

The purpose of this form is to create a user account with the appropriate level of access to use Vertere. Vertere is a software utilized by the UM for the management of chemical inventory in labs and related storage areas.

Access information

The information entered below will be used to create your account. 
Provide the details on physical location where your chemicals are stored. For example: Parker building (chemistry), floor 400 and lab/room no. 410 and 411 (Note: Do not provide the location of your office space)

Vertére role request

If your a Graduate Student (Master/PhD) or Post-doc fellow , please provide full name of your PI. 
New Inventory Set Up

Chemical Sublocation

If you are adding a new room/laboratory/storage space to the inventory, please indicate the sublocations (i.e. the specific storage location) of chemical storage present. Please select as many as necessary. 
Approximately, how many chemicals will need to be added to the inventory? If vial (small) tags are required, indicate split of the number of regular and small tags needed.
