Decommissioning devices that store data
These items store personal or sensitive data. This includes computers, laptops, multi-function printers, smartphones, and tablets. For these devices, it’s crucial that this sensitive information is securely destroyed.
Process
- Download and complete the University of Manitoba - Data Destruction form (PDF) then:
- If you are decommissioning more than one device without donating them, also complete the Data Destruction form – Schedule 1: Lot Information (PDF).
- Add your completed form(s) as attachments to an IT ticket or bring the completed form(s) to the IST Service Desk.
Important information
- If you are bringing the device(s) to the IST Service Desk, also bring the completed and signed form(s) along with the device(s).
- If requesting transportation of the equipment to the IST Service Desk (by Physical Plant), scan the completed and signed form(s) then email a PDF version to the IST Service Desk.
Uses for these forms
- Submit a request to destroy data on a device.
- Gather the necessary information about the device requested for decommissioning.
- Gather the contact information of the person who uses the device and the person who provides technical support for the device.

- Attach the completed form(s) to an IT ticket or bring the completed form(s) to the IST Service Desk along with your device.

These items do not store data or have a hard drive. This includes desktop (small) printers, monitors, keyboards, and other peripherals items. For these devices, submit a Sustainability Waste disposal online maintenance request.
