A Membership Registration Form must be completed by all customers purchasing a membership or day pass. You can also print this form and bring it with you when you come to purchase your membership. Please note that if you answer ‘YES’ to any of the questions on the health screening questionnaire, you must have your doctor complete the Medical Information Release Form (PDF) to be eligible for membership.
After reaching the maximum number of opportunities to gain access without your card, members have 3 choices;

Where the card has been lost or stolen, Recreation Services will issue the primary card holder a new membership card at a cost to the member of $12. Where the card is no longer functional due to normal wear and tear, Recreation Services will issue the holder a new card at no charge.
Members have the option to upgrade a membership within 30 days of the original membership purchase and pay only the difference. Upgrade purchase must take place within 30 days of the original membership purchase. No exceptions. Members have the option to upgrade a 1 week trial within the 7 days of the original one week trial purchase and pay only the difference. Offer does not apply to 1 month trial purchases.
A member may put his/her membership on a medical freeze, at no charge, with a letter from a physician. To qualify, the medical letter must be presented within 30 days of the patient assessment.
Members who have a 12 month term membership may place a hold for 30, 60, or 90 sequential days for a non-refundable fee of $14 per month. A member may freeze his/her term twice every 12 months for a maximum 3 months combined. Notification for freezing a membership can be given up to the day the member would like their hold to begin. A membership freeze may not be back-dated. Payments must be processed at the time of request. Freeze dates may not be adjusted once a request has been made. One membership freeze per application form. Dependent memberships may be frozen at no additional cost.
Dependent memberships are available to those children (ages 6 to 14) years of age and under, whose parent / guardian is a member and accompanies the dependent at all times.
As per the member category, all dependent members may access the facilities only when accompanied by the primary membership holder. If a dependent member is found to have accessed the facilities without the direct supervision of the primary membership holder, both the dependent and primary member may be subject to a suspension of membership privileges.
Refunds on U of M student memberships will be granted when a fee statement showing that they have withdrawn from studies at the U of M or a valid medical certificate is presented.
Refunds on staff memberships will be provided when the employee displays a Record of Employment indicating that they are no longer employed by the U of M or a valid medical certificate is presented.
UNIVERSITY OF MANITOBA EMPLOYEE PAYROLL DEDUCTION REFUND POLICY
Medical: Once a valid medical certificate from a physician is presented, payroll deductions and membership will be inactivated. No refunds will be issued.
No longer Employed: When a staff member leaves employment, the University of Manitoba will deduct the outstanding amount owing for membership fees from payroll entitlements. Once a Record of Employment from the U of M is presented the employee will be eligible for a pro-rated refund. A $15 + GST administration fee will apply.
Refunds on all other membership categories will be provided only when a valid medical certificate is presented. A $15 + GST administration fee will apply.
To continue to receive the Student Forever Membership rate, all renewals must be made on or before the Student Forever Membership plan expiration date. No exceptions.
All declined payments, including cheques and credit card payments, will be subject to a NSF fee of $30.00 + GST.
