Become familiar with your health and/or dental benefits before you need to use them! Not all treatments are covered under your plans and some treatments may not be covered at 100%.

What to Expect at the Clinic/Hospital
Some medical clinics, dental clinics, and hospitals will send the bill for your treatment directly to the health insurance company. Other clinics and hospitals will expect you to pay for the treatment at the time of service. In this situation, you will need to send an insurance claim form and the receipts for your expenses to the insurance company. When booking your appointment with the dentist or doctor, you may ask about the policies of the clinic/hospital.
When going to a doctor or dentist, be sure to bring your health card and photo identification and claim forms with you.
Health Cards:
Only the MISHIP provides a health card. If you do not receive your MISHIP health card in the mail within the first month of your classes, contact the Registrar’s Office for assistance.
Claim Forms:
Claim forms for the MISHIP are available on-line at: http://www.miship.ca/. Forms are also available at the Registrar’s Office, room 40 University Centre.
Claim forms for the UMSU Health and Dental Plan are available on-line at: http://www.umsu.ca/. Forms are also available at the UMSU Health and Dental Plan office, room 100 University Centre.
Making a Health or Dental Insurance Claim
If you are required to pay for your treatment, you will need to send an insurance claim form and your receipts for the treatment you paid for to the insurance company. If you send the original receipts, be sure to make a photocopy for yourself. The insurance company may not return the original receipts to you.
After you send the claim form and receipts, you should receive a cheque in the mail for the portion of the treatment that the insurance company will pay for. Depending on the treatment, you may be reimbursed for 100% of the cost or for a percentage of the cost.
