As a newly admitted student you are required to carefully read and review the information below and to submit the required forms to the Faculty of Social Work. Forms can be filled out online and emailed or printed off and mailed directly to the Faculty of Social Work. Please find the address below.

a. Welcome Package - for students admitted to the Distance Delivery Campus
2. REQUIRED FORM (FOR STUDENTS ADMITTED TO ALL PROGRAMS) - The following form must be returned to the Faculty of Social Work within 20 days of receiving your letter of offer.
a. An official Adult Criminal Record Check, Child Abuse Registry Self-Check and/or Vulnerable Sector Check – required (see information on how to obtain these documents) - Form
