Credit Recital Program Submission


Please note:
* All program information submitted here must have already received approval rom your major practical study teacher and your prospective jury.
* This form must be submitted a minimum of 3 weeks prior to your recital
* Complete ALL necessary fields on this form! If any are left blank you may be contacted about this and your program will not be processed until the details are complete.
* Type ALL names EXACTLY as they should appear on the program.
* It is recommended that you read through this form CAREFULLY to ensure you are prepared with ALL of the needed information before starting this form -- once submitted you are unable to go back in and edit the information
* Be aware of all necessary foreign characters in your repertoire and composers' names, eg. ö, ä, è, é, etc.  If you are unsure of some, don't worry:  indicate this in the "other info" box at the bottom and we'll help you out

Program Notes & Song Translations: (Undergrad/Post-Bacc - optional; Graduate - required) If you submit brief program notes or song translations here, they will be printed in your program. If you are not ready to submit them here by the deadline (3 weeks prior to your recital date) you may still print them on your own separately and bring them with you - the Stage Assistant will hand them out along with your program. Be sure you have already proof-read and edited your program notes before pasting them into this box. (You cannot return to this form later to input or edit these)

 

Repertoire: (Read these directions CAREFULLY!)
Input 1 piece (including any movements of that piece) in each box below. Include all of the following:
* COMPLETE title of the piece, including catalogue numbers (Opus, K., BWV, etc.) AND the title of the major work if your piece is an excerpt from something larger, for example, a song from a song cycle or an area from an opera
* ALL movement numbers and titles that you are playing from the work
* Composer's FULL name and their birth/death dates - you MUST include these dates - no exceptions!
* Full names of any arrangers or transcribers
* Full names and instruments or voice types of any assisting musicians performing on that piece other than a collaborative pianist that you named above
* Use all proper foreign characters (eg. ö, ä, è, é, etc.  If you are unsure of some, don't worry:  indicate this in the "other info" box at the bottom and we'll help you out )
* You MUST enter your repertoire IN PERFORMANCE ORDER.
