A post hospital discharge information session for patients with a cardiac diagnosis

 
The first several weeks after being discharged home from the hospital can be difficult. The purpose of the Heart Fair is to provide information and knowledge related to heart health including general information related to managing cardiac risk factors, nutrition, activity, emotional health, exercise, medication management and cardiac rehabilitation.

A variety of health care professionals participate in the heart fair and may include Registered Nurses, pharmacist, clinical dietitian, psychologist, a case manager from the cardiac rehabilitation program and a representative from the Heart and Stroke Foundation of Manitoba. Written patient education materials related to these topics are available at no charge.

The Heart Fair is held once a month at St. Boniface General Hospital from 2:00 pm - 3:30 pm in the Hospital Auditorium (Room # DG030). Upcoming sessions are scheduled as follows:

February 16, 2012 - 14:00 to 15:30
