Please find below the necessary forms to apply to the Standardized Patient Program. You will need Acrobat Reader to view the forms. If you do not have Acrobat Reader, you can download it by following this link: Download Adobe Acrobat Reader, Print the forms, fill them out by hand, and send them back at your earliest convenience, either by fax 977- 5682 or regular post to:

If you have problems downloading the forms or cannot read/ print them, contact the Recruitment Office by e-mail at webstert@cc.umanitoba.ca, or by phone at 480-1307, with a fax number and/or mailing address and you will be sent the necessary forms in hardcopy.

Please also be advised that filling out and returning these forms does NOT mean you are hired; it DOES mean that your information will be added to our database. Then, if any suitable roles come up (and the Standardized Patient Program is growing all the time!) you will be contacted for training/hiring.
