Assignment Return Form

Every assignment that is submitted to Distance and Online Education must be accompanied by a completed return form (unless otherwise noted in your course website).

The Assignment Return Form is available for download as a Word document. To download, click on the link to open the document or right click and "Save target as" to save the document to your computer.

If you are submitting by fax, mail or in person, the Assignment Return Form is also available for download as a pdf at http://www.umanitoba.ca/distance from the Forms link in the menu on the left of the screen. Please note that you can complete and print the form but you cannot save it.
