Current employees and students do not need to fill a form to get their computer accounts.
They can create their accounts by visiting iridium: the U of M Computer Accounts manager.

• Departmental Function Account (pdf)
  For an account dedicated to a specific departmental function or role

  For individuals needing department sponsorship

  For staff who will become eligible for a computer account shortly

  For an individual requesting access to someone elses U of M Accounts or data.

How to use the interactive forms
The forms which are in Adobe Acrobat pdf format can be completed online except for signatures. Please print them to add signatures and deliver to the Computer Accounts Office as described on the form.
(Adobe Acrobat is available from Software Express.)

Sample cases needing an individual sponsorship
Department administrators may want to sponsor accounts for the following individuals if they are not recognized by the claimid program:

• a volunteer, contract worker, visiting academic, or sessional who is engaged or supported by your department but who is not currently a UofM employee
• an Extended Education non-credit student who is not registered in any UofM credit courses
• a joint program student who is registered in the current session at another institution in a program that is run jointly with the UofM, for example, Nursing-RRC and Education-RRC
• a student who needs access to your CNS Novell container
