PMC2000 Make a Request Help

The Make a Request page allows you to inform the maintenance department that a repair job or other service is needed.  You should complete this form and supply as much information as possible.  The more information you provide the better the maintenance department may respond to your requests. 

You may supply the following information.  Fields which display in bold text on the request form are required.

Select a Campus
In order for the Maintenance Department to know how to process your request you need to enter a valid Campus ID.  This ID will be supplied to you by the maintenance department and should be entered exactly as specified.  If you don't enter a campus, the default Fort Garry Campus will be used.  Depending on how the maintenance department has configured the PMC2000 software this may not be desirable.

Your Name
You should enter your name in this field. This is important for the maintenance department, as they may need to contact you by phone if they need more information and want to follow-up when the request is completed.

Your Phone
This is your phone number, including 3 digit area code. This is important for the maintenance department, as they may need to contact you by phone if they need more information and want to follow-up on your request.

Your Room Number
This is your room number (office location), if necessary, for the maintenance personel to find you for more information.

Your Email Address
Your email address where any status updated may be sent.  This is required and should be a valid email account.  The quantity and type of status updates depends on the notification selection you will make (see below).

Details
You may enter as much information about the request as you need. The more information you enter, the better the maintenance department can respond to your request. You should include a detailed description of the problem, its location, and any additional information, which may be helpful to the maintenance department.

Short Description
This is a short description of the problem. It is optional, but if specified, this will be the first item the maintenance department will see when reviewing incoming requests. This field is limited to 50 characters.

Your Reference Number
Each work request may contain a reference number. This can be any value that will help you find the request in the future. If you are using pre-printed and pre-numbered forms, or wish to maintain your own list of work request numbers, you would enter it here. You may later search for requests via this number. This may contain numbers or letters up to 20 characters.

Safety Issue
If this work request involves safety issues, you may want to mark the request as a Safety Issue. This may prompt special attention in the maintenance department. Please only mark truly urgent or safety related issues as such. The maintenance department may provide additional recommendations for classifying a request as a Safety Issue.

Building
This is the building where the work request will be completed. This will help the maintenance department locate the specific problem.

Floor
This is the floor in the above building where the work request will be completed. This will help the maintenance department locate the specific problem or to specify the area of the requested work.

Room
This is the room number where the work request will be completed. This will help the maintenance department locate the specific problem or to specify the area of the requested work.

Notification
This option allows you to specify what type of notifications you want to receive from the maintenance department for your work request(s). Notifications can be sent to you automatically when a request is received by the maintenance department, when it is accepted or rejected, and when your request is completed.

You may select from the following notification options:

  • None - The maintenance department will not automatically email you any updates to this request. You may still use the Check on Request page to check on a request's current status at any time. With this option you won't be informed of the Work Request ID, without this ID you will need to use the detail search page to check on the status.
  • Request Receipt Only - You will receive one reply email message from the maintenance department only when your message is received by the department which will include the Work Request ID.  This will not indicate if your request has been accepted or rejected.  With this ID you can use the simple search page to check on the status.
  • Request Completion Only - You will receive one reply email message from the maintenance department only when your request is completed. You will not receive a message if your request is rejected by the maintenance department. With this option you won't be informed of the Work Request ID assigned to your request - without this ID you will need to use the detail search page to check on the status.
  • Request Receipt, Acceptance or Rejection - You will receive two reply email messages from the maintenance department. The first will indicate that your message has been received. The second will indicate if your request was accepted or rejected by the maintenance department.
  • Request Receipt, Acceptance, and Completion - You will receive three reply email messages from the maintenance department. The first will indicate that your message has been received. The second will indicate if your request was accepted or rejected by the maintenance department. The third will be sent when the request has been completed or if it is cancelled.
  • All - This option provides the highest level of feedback about your request. You may also be informed when work on your request is started.

Note: The notification level is maintained throughout the life of the request. Once you save the request the level may not be changed. Regardless or the notification option selected, you may use the simple and detail check on a request pages to get updated information about the request emailed to you.

Press the Submit button to save your request and send it to the maintenance department.

Press the Reset button to clear the request form and start again.
