ADVISOR CHANGE

There may be an occasion when a change of an Advisor or Co-Advisor is necessary.   In these cases, the change must be approved by the 

  • Student's Current Advisor
  • Student's Current Co-Advisor (if the student has a Co-Advisor)
  • Student's New Advisor
  • Department

 

FORM:  Please use this form for     BME Advisor/CO-Advisor Changes  

 

 

Ph.D. COMMITTEE CHANGES

  • ADVISORY COMMITTEE:  Any changes to the advisory committee, including the advisor/co-advisor, must be approved by the Faculty of Graduate Studies

    Must be approved by the
    • Advisor
    • Co-Advisor (if applicable)
    • Department
    • Faculty of Graduate Studies
  • EXAMINING COMMITTEE:
    • The Dean of the Faculty of Graduate Studies must approve changes in the membership of the examining committee.
    • NO CHANGES are allowed to the Examining Committee AFTER the thesis is submitted to the Faculty of Graduate Studies.
    • Should the thesis not be submitted for examination within 12-months after the appointment of the examining committee, the committee appointment will lapse and the process will need to start anew

  • Cross out the Committee Member being replaced
  • Obtain the signatures of the
    • New Committee Member
    • Advisor
    • Co-Advisor (if applicable)
