How can I confirm that my benefits information is up-to-date?
You can sign on to Jump, click on the HR tab and follow the link to "Employee Self Service" (ESS). Review your benefits, dependents and beneficiary information in the "My Benefits" tab.
There is no beneficiary listed on the "My Benefits" tab in the Employee Self Service (ESS) Portal. What does this mean?
This probably indicates that you have not listed a beneficiary on your group insurance application form. You should contact the Staff Benefits Office and arrange for forms to be mailed to you.
Can I name more than one beneficiary?
Yes. Let the Staff Benefits Office know that you wish to name more than one beneficiary and forms will be prepared and mailed out to you.
Can I name my children as beneficiaries if they are minors?
It is advisable that you do not. In this situation, should you die prior to the date the children reach legal age, the proceeds could not be paid out until they are of legal age. If children are minors, you should name your Estate and make provision in your Will to appoint a Guardian to handle the proceeds.
One of my children is not listed as an eligible dependent. Are they covered under my insurance?
No. You should contact the Staff Benefits Office to complete a form to add them as an eligible dependent.
Do I need to take any action if there is (are) dependent(s) listed who are no longer eligible?
Yes. You should contact the Staff Benefits Office to complete a form to delete ineligible dependents.
Why does the ESS - My Benefits indicate my coverage as couple when I am a single parent with one child?
Couple for insurance purposes means two people. This could be a single parent with one child.
Can I convert my insurance if I terminate from the University of Manitoba?
Yes, you have 31 days from the date of termination to convert your life insurance and/or dependent life insurance on your spouse to an individual policy with Great-West Life. More detailed information is provided with your termination package. If you continue to be associated with the University, but cease to be eligible for benefits, you should contact the Staff Benefits Office for conversion information, keeping in mind that the conversion privilege, if desired, must be exercised within 31 days of termination of insurance.
How can I increase/decrease my optional insurance coverage?
Contact the Staff Benefits Office and forms will be mailed out to you for change.
I've noticed a change in premiums for group insurance (Group Optional Life) on my pay stub. What does this mean, when I haven't changed my insurance?
This means that you have entered into a new insuring age group which has higher premiums. This change occurs in July. Premium rates are also adjusted in July of each year according to the experience of the plan.
How do I find out if my dental claim has been processed by Blue Cross, or if a particular dental service is a covered procedure?
Contact Blue Cross at 204-775-0151 and provide your six digit employee number with prefix '8' (i.e. 8-xxxxxx) and policy number 67000 for full-time staff members and 67025 for part-time staff members. If your inquiry relates to whether a particular procedure is covered, you will also need to provide the dental code obtained from your dentist.
How do I find out if my Supplementary Health claim has been processed by Great-West Life, or if a certain medical supply or service is a covered expense?
Contact Great-West Life at 1-800-957-9777 and provide your six digit employee number and policy number 20778. If your inquiry relates to whether a particular service or supply is covered, you will need to describe it.
How can I obtain a claim form?
You can download the (claim forms) from the Staff Benefits website, contact the Staff Benefits Office at 204-474-7428, or pick them up from the Staff Benefits Office, Room 180 Extended Education Complex.
