Application Forms

Electronic copies of the application forms are provided here for you to download and print on your own computer. If you have trouble viewing or printing these forms, you can get paper forms from the Human Resources offices.

The Adobe Acrobat Reader software is required to view and print the application forms.
This software is available for free download from the Adobe Acrobat web page.

To download and install Acrobat, follow the steps below (please read all the steps before going to the Adobe web site, or print the instructions so you can refer to them later).

  1. Go to the Adobe Acrobat download page.
  2. Under "Step 3" (which is marked with a large red number '3'), select the Acrobat version and language you need, and the type of computer you are using.
  3. Click on the "Download" button, which is located further down the page.
  4. On the next page, scroll down until you see "Get the Reader now! Just click on a Download Link." Then click on one of the links shown to begin downloading Acrobat (it's usually best to select the link whose location is closest to your own).
  5. A "Save Dialog Box" will appear. Select a location to save the Acrobat install file. It doesn't matter where you save it, but you should make a note of the place you save it, because you will need that information to complete the installation. Click the "Save" button to begin downloading.
  6. When the download is finished, exit your web browser (select "Exit" or "Quit" from the file menu).
  7. Locate the Acrobat installation program (the file you just downloaded) on your computer.
  8. Start the installation program by double-clicking its icon.
  9. Follow the on-screen prompts and instructions to complete the installation. Make a note of the location where the program is installed.
  10. When Acrobat is installed, start your web browser again and return to this web page.
  11. Click on a link to one of the application forms.to view or print the application forms. The first time you do this, you may see an "Unknown File Type" dialog box. If this occurs, click the "Pick App" button. In the dialog box that appears, locate the Acrobat program file (the location you made a note of in Step 9, not the installation file from Step 5). Click OK. You will not need to repeat this step the next time you try to view an application form.
  12. If you followed these steps correctly, the application form will download and appear in an Adobe Acrobat window, where you can print it using the print command on the file menu.
